TERMS & CONDITIONS

  1. Samples can only be sent to a valid dental practice address.
  2. Requests are limited to one per Colgate Oral Health Advisor.
  3. Individual sample offer supplies are limited and will be based on the individual offer’s available quantities. Incorrect or incomplete requests will not be fulfilled.
  4. Coupons are intended to be provided to patients.
